1 STATE INNOVATION PLAN

2 Purpose To test whether new payment and service delivery models will produce superior results when implemented in thee context of a state-sponsored State Health Care Innovation Plan. Plans must Improve health, improve health care and lower costs through a sustainable model of multi-payer payment and delivery reform Be dedicated to delivering the right care at the right time in the right place

3 Funding Center for Medicare and Medicaid Innovation (CMMI) 25 States to be awarded design funds Average award $1 to $3 million Delaware’s application: $4 million

4 Project Timeline Project period: December 2012-May 2013 Notification of award: first week December All activities to be completed by May 2013

5 Proposed Approach Six work streams Delivery System Transformation Payment Model Transformation Data/Analytic Infrastructure Analysis/Enhancement Workforce Policy and Purchasing Alignment Population-based Approaches to Health Promotion

6 Project Goals Develop a State Innovation Plan that, when implemented, results in Improved health Integrated/efficient health care system Lower costs Measures and incentivizes performance and outcomes at all levels Incorporates existing activities that work across the health care system including Existing CMMI projects CCHS AI DuPont Hospital for Children Innovative delivery system pilots (e.g., PCMH) HCC workforce CHPDP

7 Delaware’s Advantages Population, provider and payer landscapes that enable the proposed models to impact the majority of the market; Advanced health information infrastructure to support care coordination and performance measurement; Sophisticated capabilities to integrate and analyze data; State health policy leadership, innovation and experience; Strong educational alliances supporting workforce transformation; Extensive stakeholder networks and tested engagement strategy; Strong working relationships between state and private payers.
