6. WHAT WE HAVE ACHIEVED Adopted a Team approach and not focused solely on the Community Health Nurse Locality implementation groups with public involvement Staff adopting new roles to meet the needs of the community

7. WHAT’S NEXT? First cohort of staff will undertake Transitional and ongoing education in February Roll out of training and development at locality level to whole team Positive development of Community Nursing Teams
