2 The Area Diabetes Clinical Stream Leadership Group What is the purpose of the leadership group? “The Area Diabetes Clinical Stream Leadership Group is the peak group in relation to diabetes care within Hunter New England Health. It will lead and overview the implementation of the Area Diabetes Services Strategic Plan”

3 What is the role of clinical stream Facilitate strong relationships – across facilities, services, ACN/ACS and external partners to support service clinical delivery Professional development - to enhance peer support and professional training and education for staff Information - to facilitate the provision of information for service providers and consumers and their families to support access to appropriate care Clinical practice improvement - to coordinate the development, review and use of appropriate clinical practice guidelines across relevant areas
