2 Remember, goal of coaching in QI A team should be able to: – measure and/or use data to do QI (PM) – improve care (QI) – sustain the work over time (QM) Goal of monitoring is to – give you feedback on progress to these goals – Identify areas where more work is needed – Identify areas where you can/should be changing

4 Goal of monitoring a QI project (or QI program) To learn whether the project is on track, or is likely to fail If the project is not achieving the intended results, to understand why To provide guidance, support, and encouragement (or push) to the project team Adapted in part from NQC

5 The changing role of the coach over time What you do will change over time as the QM program or a specific QI project progresses If not, the clinic may have become stuck You will only know if you monitor

6 The changing role of the QI coach: QI projects Before the Project – Did they measure? – Support team leader and overall QM team in good process to choose the gap Data driven Team-based – Teach needed tools and skills to team PDSA, measurement – Make sure the project aims are clear Adapted from the Team Handbook

7 The changing role of the QI coach: During the project (1) Talk with team leader to check on progress of planning and implementation – Has the first PDSA been tried? – Have they refined and tried again or expanded? – Did they re-measure? Assess how is the team functioning – Help get the team unstuck during meetings Meet with team and leader together for review – Are things moving along, what needs to be changed? 8 The changing role of the QI coach During the project (2) Discuss trends and chance of success. If things aren’t going well, why? – Where can you coach? Remember the coaching steps – Remember to provide positive feedback If things are going well, what is next? – How can this be integrated and sustained? – What is the next area for QI? – How can the success be shared with others? 9 The changing role of the QI coach After the project – Review with team final results and next steps – Help leader and team develop the messages to disseminate the results to the rest of the clinic – Help team move to integrate and sustain the improvements – Coach them to identify next QI project(s) This cycle repeats over time with gradual decrease in coach’s support. 11 Monitoring the QM program Is the team meeting regularly – Who is coming? – Are they documenting (minutes)? Are they choosing and completing QI projects – Bringing in other clinic members into projects? Are they using data? Do they have an overall plan and goals? Are they communicating and sharing with the rest of the clinic and leadership? Adapted in part from NQC

12 Brainstorm-small group and share Assign a note taker Discuss the following – What tools have you used or seen used to monitor QI programs or projects – What (other) tools would be useful 15 minutes, take notes and be ready to report back
