2 One online form Report work-related injuries Report injuries to visitors, guests, students, volunteers, and others

3 Supervisors or HR partners log in with MyID & password

4 Select the New Injury Report button

5 Input basic information

6 Describe the incident including any witness contact info

7 Describe the injury and who is reporting

8 Work-related or not? May print at this time. Press SUBMIT when complete.

9 After you submit the injury report (work-related)
