Documentation Documentation is needed (in addition to the narrative on DCA 4 and DCA 5) to substantiate needs and proposed solutions The severity of need is determined by the extent to which the need affects the health and safety of low-to moderate-income residents. Documentation Preliminary Engineering Report / Preliminary Architectural Report—see list, p. 50-52 of Manual Photos in color, dated, keyed to a map Views of entire area and specific concerns Maps—USGS topo; See DCA 12 for list Project Cost Estimate – ED: Size & Capacity “Source & Use Statement”

Letters of Support Community Development Local Government match commitment / operation and maintenance pledge Citizen / Organization letters—individual, with specific details are best Citizen survey comments Economic Development Company commitment letter / All participants Bank commitment letter - Loan

Documentation Community Development Work orders / repair records Fire department records or letters Test results: fire flow tests, smoke tests, video of sewer lines, etc. Economic Development Job descriptions – title, description, and wage range Company background/financials Public ownership
