2 You said…we did Continuum of Adult Care You said…  Work towards integration  Commission better community and improve primary care services  Commission better mental health care (especially dementia) We have (or we are)…  in a pre-procurement phase for Community Health Services for North Somerset—a key focus is for better integration of services  Commissioned the British Red Cross to provide an assistance programme—transport home and resettlement for up to 2 hours for vulnerable patients  Partnership working to commission a Parkinson’s specialist Nurse.  Implemented a Dementia ‘roadmap’ - high quality information and signposting to appropriate support at the appropriate stage of the condition

3 You said…we did You said… Planned Care (& Medicines Management)  Commission high quality planned care services  Provide choice and keep within national targets for wait times We have (or we are)…  Continuing our programme of individual funding requests  Consistently delivered on the 18 week ‘referral to treatment’ target  Commissioned the ‘map of medicine’  Maintained the spend on medicines at the lowest quartile in the country  Supported each GP practice with a Medicines Management Pharmacist to ensure safe, high quality prescribing to improve patient understanding and concordance of their medication

4 You said…we did Urgent Care You said… We need  Better information about NHS 111  Better information about the Minor Injury Unit in Clevedon  Improved Ambulance response times  Assurance of good /appropriate access to Emergency Care We have (or we are)…  Implemented a successful winter awareness campaign ‘think ABC before A&E’  For 5 months, achieved the 4 hour performance target for Emergency Care in Weston  Invested £4.8 million in enhancing community care capacity and improving the acute ‘system flow’  Working with the Ambulance Trust to improve response times

5 You said…we did You said… Children and Young People’s Care  Ensure there is less waiting time for specialised assessments for children and young people  Commission better services through transition for young people We have (or we are) …  Made a decision to re-commission Specialist Children’s health services – today is the launch of our ‘engagement conversation’  Significantly reduced waiting times for first appointments for children’s community paediatrics  Refreshed the multi-agency protocol for transition; and  Prioritised transition in our delivery plan for 2014/14

6 You said…we did Primary Care You said… We need…  Better access  Better patient information to self care  Better out of hours care We have (or we are)  Supported a successful bid to the Prime Minister’s Challenge Fund  Promoted NHS 111 and ABC to support appropriate access  Commissioned the new out of hours provider who is delivering a high quality service that has consistently received good feedback  Commissioning a new outreach to support Central Ward as part of the Weston-super-Mare Integrated Team

7 You said…we did Equalities and Engagement You said… We need  Better awareness of equalities issues  More opportunities for engagement We have (or we are)  Recruited and trained an Expert Equalities Group to support the CCG with health planning  Developing a new North Somerset CCG website  Continued to build and develop a positive relationship with Healthwatch North Somerset
