PLAN OF STUDY (POS) - BSDH Courses in blue are required prerequisites and will be calculated for admission into the clinical program as application gpa and science gpa. Last advisor who reviewed your record. Required clinical courses once accepted into the clinical program. Placement Test Scores Required courses due to Placement exam results Replaced with PHSL-P 261 and P262

Grade Replacement Policy -You may grade replace three courses or a maximum of 10 credit hours. The second grade will be calculated into your cumulative-grade-point-average. -You must request Student Services to process paperwork after you have received your grade and successfully completed the repeat. Email, call or stop in. -Registrar’s Office processes paperwork…can take up to 2 weeks for processing depending on time of year. -For more information refer to IU South Bend Bulletin. Policies You may withdraw from a course online (Onestart) through the 10 th week of a semester. Always check the academic calendar. You do not need faculty or advisor signature. There is no refund. You will receive a “W”. Affects of withdrawing: If receiving Financial Aid, withdrawing may impact your aid package and scholarship awards. Always check with F.A. prior to dropping. 21 st Century Scholars must complete 30 credit hours during the academic year. May place you “out-of- sequence” or push back your application date for admission into Dental Hygiene Program.

General Education Electives COMMON CORE ELECTIVES (190 & 390/399) Please check the faculty General Education Website at: https://www.iusb.edu/general-educ/ for latest listing of courses offered. https://www.iusb.edu/general-educ/ -Remember, one of your common core electives must be at a 390/399 level. Your Common Core Electives are N, T and B – one of which must be a 390 or 399 – Junior-level course. All other General Education descriptions including approved courses can be found at the above web site.

For the BS Dental Hygiene degree program, the science sequencing of courses remains the same: Completion of MATH-M 107, College Algebra or Math Placement Test at Level 4. CHEM-C 101/121, Elementary Chemistry I with lab. Exception: if you graduated HS within past 3 years and had one-full year of HS Chemistry (not integrated CHEM/Physics) and you can remember basic principles, you may bypass CHEM-C 101/121 and move to… CHEM-C 102, Elementary Chemistry II, 3 cr. hrs. Offered spring, summer I and fall. Science Sequence

CHEM-C102 is the prerequisite requirement for PHSL-P 261 (A&P I) and MICR-M 250/M255 (Microbiology). PHSL-P 261 is offered in spring and fall; MICR-M 250/M255 spring, summer session II and fall. BSDH students must take MICR-M250/M255. If you enroll in M250/M255 and need to withdraw from M250, you must withdraw from both. PHSL-P262 is offered in spring and summer I. November 3-8 Are You Ready to Register for Spring 2015?

Set-up your shopping cart Look at your Plan of Study (POS) and determine list of courses you wish to enroll for Spring 2015. REFER TO YOUR REGISTRATION MAILING! Check for any holds which might prohibit registration. Take care of them! Choose your classes (use the blank scheduling sheet we provided in the mailing). Make sure you have the class number so you can set-up your Onestart registration shopping cart.

Look for Negative Service Indicators “holds” which might prohibit registration. All “holds” must be taken care of 24-48 hours prior to registration.

Summary Check Onestart Account for the following: Holds : Financial, Immunization, Advising. Contact the appropriate office for resolution. Check for your registration appointment. This is your first opportunity to register for classes online. It is not an advising appointment. Will provide time and date when your ability to register is active. See Student Services with your questions. You can also email cbehrend@iusb.edu, saambler@iusb.edu or jgilroy@iusb.edu cbehrend@iusb.edu saambler@iusb.edu jgilroy@iusb.edu Set-up your shopping cart with proposed classes. Meet with an advisor to review your selections. Register on-time. This will insure first and best opportunity to secure seats in the courses you have selected. If you physically do not know how to register, choose your classes and on your time/date go to the Gateway Information Center, Main Lobby of the Administration Bldg. and they can assist you with inputting your class selections into the computer.
