Access Points/ Housing Provider Support Work Group Purpose: Provide better support to PSH providers and clients to utilize housing resources effectively and efficiently

Work-to-Date Universal Application and Documentation – HUD representative contacted who is willing to work on written guidance – Sample universal application obtained that is being utilized in NY PSH Monthly Meeting – At the September PSH Provider meeting, requested feedback on PSH process and improvement for the system with solutions to bring to next meeting. – In-person meeting scheduled quarterly instead of monthly meetings via telephone. Pool of providers to complete disability form – CCH has 3 providers who are able to meet with clients – Will work with other agencies on setting up similar system if needed HCBS Waiver-Supportive Services for Consumers – Presentation by Colorado Access on 10/22/2015 – Will work on a community wide training Restructuring Match and Pre-Match Queue Policy and Procedure – Group created policy and procedure, need to get feedback from Case Conferencing Group and present it to the entire CDT for approval. – More streamlined approach to updating the lists.

Next Steps Work to conduct a PSH survey to get feedback and solutions to system issues. Look at the MIF to tailor it to PSH needs Determine documentation standard for PSH eligibility Determine which organizations have the capacity to offer time to housing navigation Evaluate the type of level of services that are provided by PSH Coordinate across MH providers Modify the Housing Vacancy Form Bridge Housing
