13 Enrolling in the Online Course

14 Enrollment Agreement The techniques taught in this online curriculum can improve the quality and completeness of any application for SSI/SSDI. Individuals who complete the course are encouraged to use what they learn to improve SSI/SSDI applications for themselves or others. However, those who enroll in this course agree that the label “SOAR” will only be used on SSI/SSDI applications for persons who are homeless or at risk for homelessness. In completing the online curriculum practice case, case managers are required to do their work independently and not copy others or submit "joint" work. This is a requirement to receive a SOAR certificate of completion and 16 CEUs from the NASW. I understand and agree to abide by these conditions.

29 Tips for Success  Include information from the videos, progress notes, and 4 treatment providers  Don’t get too caught up in exact dates or whether or not you are playing yourself or the fictitious case manager; the important part is that the main pieces of information get to the right places on the SSA forms/MSR  Sometimes you won’t have all the details about the applicant’s history (e.g. dates/hours of work and rate of pay) and that’s okay; you have the Remarks Section!!

30 Tips for Success  Use the MSR template to help organize your information. Each section should be addressed.  For the psychiatric history section, be sure to address all treatment providers that aren’t covered in other sections of the MSR  Write at least one long paragraph for each section of functioning  Review the sample MSRs and sample completed SSA Forms! http://soarworks.prainc.com/article/sa mple-medical-summary-reports

32 Keys for Organizing Information  Remember that information is spread out throughout the course, simulating how you will receive information in real cases (bit-by-bit)  Use the SSA Forms Guide in Class 1 to review what information each SSA form is asking for

33 Keys for Organizing Information  As you gather information from the progress notes, videos, or treatment records, add it into the appropriate form or space in the MSR  This will help prevent you from having to go back and gather the information a second time  The SSA forms 16 and 8000 are uploaded in Class 4 – be sure to use information from all the classes (including Class 4) when completing these  If you complete these early, you may miss information.

34 Top Mistakes  Not including all treatment providers on the SSA-3368  Remember that jail and substance use records can contain critical information.  In the MSR, missing a connection between the applicant’s symptoms and functional impairments  Use quotes and multiple examples to show marked impairments  Review the class articles that explain what each question on the SSA forms are asking – some questions are tricky and can be easily missed if misunderstood
