Community Health Assessment Jackson County is a small county in northeast Kansas. Population of 13,240 Despite being a small community, it offers a variety of healthcare services. Primary healthcare services are provided by physicians in 4 clinics. One community hospital serves the area

Conclusion Periodic re-evaluation of community resources is necessary by all communities to keep up with the needs of their population Development of state rural health plans Successful implementation depends on policy makers, health care providers, health care leaders, associations, universities and communities.
