2 Inclusion & Empowerment Good practice includes recipients as partners in developing services that address personalized needs within a specified time frame This approach empowers recipients to be more self-sufficient and assume more responsibility for themselves

3 Holistic (Systemic) Approach Good practice recognizes the multiple needs of a recipient even if the agency can provide assistance for only one or two of those needs

4 Recipient as a Resource Good practice views the recipient as a resource who is capable of helping herself and of accessing informal support systems with assistance from the agency Professionals are facilitators in this process - not controllers

5 Case Management A case management approach helps the recipient develop personal skills and utilizes formal and informal supports Case management reduces the likelihood of fraud and duplication of services

6 Transparent Information Good practice provides transparent and clear information about:  Recipients’ rights and responsibility  The agency’s role  Personalized planning based on input from applicant during the needs assessment process
