2 Lessons Learned Update May 2013 Federal Aviation Administration 1 1 Background During October 2011 meeting, COMSTAC developed the following finding: “COMSTAC finds that it would be beneficial for the FAA to develop a process for disclosing to the industry pertinent data from reported safety critical anomalies, mishaps, incidents, and precursors where relevant to current and future operations. Such a process needs to protect proprietary data and comply with relevant export control policies whiles still fostering the continuous safety improvement of the industry” Subsequently, the House Subcommittee on Space and Aeronautics asked AST: “Do you have a model in mind for a system to exchange lessons learned data between government and industry?”

3 Lessons Learned Update May 2013 Federal Aviation Administration 2 Update to October 2012 COMSTAC Currently working to update internal lessons learned database for safety trend analysis to make it ready for access for COMSTAC members Adaptation of AST’s STAR database of > 5,600 orbital launches worldwide We are updating and modifying the database to make it easily searchable We are in the process of setting up complete, easy to access standard reports Reports will be easily customizable STAR Database has been modified significantly, adding fields useful for trend identification The information in the database is not proprietary Tauri Group gathers the data from a variety of public sources 2

4 Lessons Learned Update May 2013 Federal Aviation Administration 3 3 Update - Continued STAR database will be rolled out to the COMSTAC members (and other select stakeholders who request it) in the near future COMSTAC members, et al will provide a username and will be issued a temporary password Login instructions will be available on the AST website STAR database will open initially as a beta test for a trial period of six months During that time, AST would appreciate feedback from COMSTAC members on how the system could be improved AST will consider all feedback and make improvements as applicable Goal is to evolve STAR into a current lessons learned database, with inputs from industry Beta test will help determine feasibility, and suitability of STAR to satisfy industry needs
