Definition of Reablement According to CSED 2007:- –“ Services for people with poor physical or mental health to help them accommodate their illness by learning or relearning the skills necessary for daily living.” –Reduces need for ongoing commissioned service. –Up to 50% of users no requirement for ongoing support.

The Teams A short term service AP In reach team will take referrals and carry out first visit establishing the initial Personal Support Plan AP will constantly monitor and review service user progress AP will carry out reviews with the service users and bring their findings to the review meeting AP In reach team will fast track referral for equipment and may procure certain pieces of equipment directly AP will contribute the individuals PSP as an on- going process

What does the model look like? Initial Assessment carried out by commissioning teams and outcomes identified Home Support provided on a day to day basis Weekly goal setting with service user to achieve their stated outcomes; carried out in conjunction with therapists Week 3 or 4, if the service user is still receiving reablement service, will consider future needs of service user with social work input. Consideration to be given to Direct Payments and Assistive Technology Final review will produce an outcome focussed Support Plan with recommendations for on-going support Exit strategies with AP In reach linking with voluntary and statutory services for a comprehensive discharge.
