1 Turner Elementary School Health Based Clinic (AAPHC) “Welcome to the Home of the Eagles” Principal: Dr. Deborah H. Jones Assistant Principal: Mr. Calvin Poole

2 Partners in Education and Healthy Living Nurse Angie Barber and Dr. Deborah Jones share a moment after receiving gifts for students.

3 Dental Services of AAPHC Students are now able to visit the Dentist without leaving school and without their parents. Minimal class time loss in instruction is our goal.

4 Changing the school Culture in Healthy Eating Habits Grandparent’s Day was planned carefully to include a healthy breakfast instead of snacks full of sugar and fatty ingredients.

5 Mr. Sharpe- Teacher of the Year Classroom We have created the healthy living Culture through out the building.

6 We include parents in the Process of Healthy Lifestyle Changes

7 Students Enrolled in the Health Center We have a total of 445 students enrolled in the Health Center which is 83.3 % of our student population. Number of Student Health Checks Quarterly are: – 1 st Quarter 2014-2015- 158 – 2 nd Quarter 2014-2015- 78 – 3 rd Quarter 2014-2015- 110 – 4 th Quarter 2014-2015- 19 – Total Checks for 2014-15 was 365 which is 68.35% of our student population.

8 Health Maintenance 2014-2015 Number of Students served aged 0-6 was 119. This reflects early detection of medical issues that may impede learning. Many students start school with eye sight problems that go undetected as preschoolers. Number of Students served aged 11-18 was 186. This number includes many students that are siblings as well as students needing physicals to play local sports in the district.

9 Immunizations Performed at AAPHC The School Health-Based Center performed 117 immunizations during this school term (2013-2014). This assist the parents and the schools in helping to make the enrollment process of kindergarten less stressful and delayed.

10 Attendance Healthy Students have good attendance. Students often were pulled out of schools for appointments and would not return to school. The Health-Based Clinic allows for the students to be seen without parents with minimal instruction time lose.

11 Behavioral Health Support Number of students screened for behavioral health problems: 2013-2014- 334 2014-2015- 521 Number of students referred for intervention: 2013-2014= 56 2014-2015= 73

12 Obesity in Children at Turner Elementary Number of students screened for obesity= 2013-2014= 446 (488) 2014-2015= 655 (534) Number of students Identified with BMI over 85 th percentile and receiving intervention= 2013-2014= 143 2014-2015= 219

13 Asthma in Students at Turner Elementary Asthma Data for Students Attending Number of Pts. w/Asthma= 79 Number of Pts. w./Severe Asthma= 10% Number of Pts. w/Action Plans= 100% Percentage on Pharmacy Meds= 99% Medical attention, education, and healthy prevention techniques are all provided to support the Asthma patience.
