7.  This is the form that your teachers will use if there is a situation/infraction in the classroom or if you need to leave the room.  This form must be kept in your assignment book.  At the end of the 9 week, any student that has 15 or less marks will be invited to the 9 week incentive.  If a student gets a total of 15 or more marks, he/she will be called into Team to discuss how the issue can be corrected and possible disciplinary actions. 15 AND OUT FORMS
