Get Illinois certification immediately when you graduate! Saves you from potentially having to take additional course should the program, ISBE or state law requirements change. This is extremely important! Provides permanent record of certification Other states may not certify you without Illinois certification first Applying with the COTE (in and of itself) is no protection against changes!!! Claim certificate!

◦ Fill out the form completely (if there’s an entry, there’s a reason for it). ◦ Turn in to Cheryl She will turn it in to us when she has all the documentation she requires of you.

Entitlement Request Form Name Must use name that appears on Banner We have to recommend you based on name that appears there. Name changes? Change Name in Banner Contact us Mailing address Most permanent address

Date of Birth Must have in order to process (mostly just to make sure it’s you). Month/Year Program Completion This should be the month/year you will be completing ALL necessary requirements for certification. Degree Put type of degree you are earning (must have master’s)

Email address. Important!!!! This is where we will notify you to go online to complete certification process May use any email account Your UIUC account eventually “goes away” so it may not be the best choice. Check regularly Should keep this email account from filling up and having e-mail bounce back to sender (us) Check your spam and junk folder too Phone number # where we can reach you w/questions

Read small print Print name, sign, and date form Verification of Experience Form IGNORE IGNORE IGNORE (This is for administrative folks only) Turn application in to Cheryl ◦ Unsigned applications will be returned ◦ Retain a copy for yourself

Final audit is based on SPSHS evaluation provided by Cheryl. If we try to contact you, please respond Make sure courses listed match transcripts Non-UI courses must have transcripts attached Must come from school or be sealed by school Nothing can be done until we have this/these documents

Requirements Needed Prior to Recommendation for Certification Appropriate test scores Letter of Recommendation from Cheryl Satisfactory completion of internship Portfolio Review Passing grades for required courses (C or better) Degree (has to appear on transcripts) Obviously this may pose problems for employment. Speak to Cheryl or my office to see if we can help.

We will notify you by e-mail that your entitlement has been processed (roughly 3 weeks after graduation) Go to Educator Certification System (we will provide link in the body of the email we send) Create an ECS account Apply for certificate Log on to Educator Certification System (ECS) Be prepared to pay with credit card Be sure to register certificate online: PRINT COPY OF REGISTRATION School districts can verify your certification as soon as you claim your certificate Certificate will arrive from ISBE ~roughly a week later

Testing www.icts.nesinc.com Required tests: limit of 5x per test ◦ 1) Basic Skills (reading, writing, grammar, mathematics) (096, or (300)), or Test of Academic Proficiency (400) ◦ 2) Speech Language Pathologist: Nonteaching (154) Can take both tests on same day Make sure SSN is correct Make sure scores are sent to UIUC (052) Keep your scores forever!

Certification Miscellaneous ◦ Before July 1 Type 73 Certificate ◦ On or after July 1, will be a Professional Educator License endorsed in Speech Language Pathologist non-teaching ◦ Certification in other States Likely based on completion of approved program and certification in Illinois May have form for COTE to fill out – Send to us!
