12 Eligibility Requirements (Pg. 2) -2016 Wellness Program Enrollment Form – Most completed in November. -If you have not filled out a form and want to participate, now is your chance. -New Hire (Hired before April 1 st 2016 must complete “All” requirements, however those hired after April 1 st 2016 will be expected to do as much as they can if signed up for the program). -Health Assessment (Must be completed March 1 st – April 1 st ) -Health Assessments will be accepted NO LATER than April 1st -On-line or Paper form -Blood Work (Deadline May 1 st ) -Associate Blood Work April 1 st – May 1 st -(Any blood work completed and submitted to One Stop Wellness January 1 st - May 1 st, 2016 will be accepted for the Wellness Program). -Blood work can be submitted by e-mail onestopwellness@epbnet.com or fax to (888)-681-6146 onestopwellness@epbnet.com

16 “Wired Up” Quarterly Agenda 1 st Quarter (Pg. 3 & 4) 1 st 10 Week On-going Health Management Program “Targeted… Glucose” (January 18 th – March 21 st ) Take advantage of the opportunity to “Target” one of your most important numbers for cold hard cash. If your Glucose is already in range, simply maintain this number. If it is out of range, simply improve it by the designated percentage. You will be rewarded for either maintaining a healthy glucose or improving it by the designated amount. Health Assessments (March 1 st – April 1 st 2016)

17 2 nd Quarter (Pg. 3 & 4) -2 nd 8 Week On-going Health Management Program “Sip, Stand & Stretch” (April 11 th – May 30 th ) Today it is estimated that the average American is only active 3hrs per day, leaving the remaining hours sedentary (sitting, sleeping, watching TV, etc.). This program is designed to help determine how many active hours per day you typically have and then finding ways to increase those active hours. Associate Blood Work (April 1 st – May 1 st 2016) Spouse Blood Work – VOLUNTARY. For Details visit www.1stopwellness.net (Click on the “Blood Work” tab under the SEWS tab for details). Spouses must register by March 1 st in order to be eligible for free blood work.www.1stopwellness.net

18 Spouse Blood Work Experimenting Voluntary (will not effect the benefits discount) Spouses only (No Children) Only spouses on SEWS insurance plan At a local LabCorp draw facility during the month of April for those who are registered Exception: Scottsville/Edmonton We'll open up 1-2 days in Scottsville/Edmonton during the month of April. Participating spouses at these locations must follow the registration instructions below. Spouses participating must register through One Stop Wellness Click on the “Blood Work” tab under the “SEWS” tab Click on the link for registration – Deadline to register = March 1 st Once registered One Stop Wellness will mail your spouse instructions and a requisition form during the month of March. Results will be mailed to the address they designate within 7-10 days after blood work is completed.

19 3 rd Quarter (Pg. 3 & 4) 3 rd 8 Week On-going Health Management Program “Lunch Matters” (June 13 th – August 1 st ) Lunch is a prime opportunity to make better choices. Are you brown bagging your lunch, if so what do you typically pack? How much are you spending if you are eating out? What choices are you making when eating fast food or out of the vending machine? These are all things that you should consider because lunch matters. Delivery of Results/Health Coaching (June 2016)

20 4 th Quarter (Pg. 3 & 4) 4 th 8 Week On-going Health Management Program “Step This Way” (August 29 th – October 17 th ) SEWS will reimburse ½ of the cost of the tracker of your choice up to $50. All you have to do is complete the 8 week program and submit a receipt for the pedometer of your choice. This is a great way to take advantage of the fall weather and “step” your way to good health. Remember… you don’t have to have a tracker to take part in this program. We will provide a manual tracking system if you want to participate without a pedometer. October – All about the Ladies… We’ll celebrate the ladies and focus on women’s health issues November – All about the Fellas… We’ll celebrate the men and focus on men’s health issues. November – “2” $1,000 Drawings. You get one entry for every non- webinar qualifying event completed. Deadline for Completing 2016 Action Items (November 1 st 2016) Reenrollment into the 2017 Wellness Program
