1 How to use this tool The young person’s Self Assessment Tool is to be filled out with your case manager or youth worker. There are three steps: 1. Read the statements about each of the areas : Wellbeing Relationships Involvement 2. Place the marker (X) in the places that best applies to you. 3. Talk with your teacher, case Manger or youth worker and decide what strategies will best help you improve in that area. Use the box section to write in what you decide. Fill in the sections below and ‘save as ‘ for personal use. To select and move your maker position you need to be in non-slide show mode To type into this assessment tool the Power Point presentation requires you to be in view slide show mode.
