How to complete an Office Discipline Referral Created by PT

Go to the district website Click on Employees

You can enter the Student ID number

OR You can enter the Last and First Name

Click Run Query

After typing in the Student ID Number or the Student Name you then must click on the drag down menu and select Discipline_Referral_Form

Click on Lookup Admin Email

Click Submit for School Approval
