Performance Goal Format [Enter the standard and indicator for focus] I will [describe what you will be doing] by taking the following steps: [list steps you will take to achieve your goal]. The goal will be achieved by [enter date] and measured by [indicate the measure you will use to show goal achievement]. The goal will be achieved by 5/31/2016 and measured by a case study.

Action Steps Action Steps are created by the professional What are action steps? Action steps explain the SLO implementation plan. Describe the group(s) that will be used for the SLOs. Identify the instructional strategies, artifacts and evidence to be collected. Describe timelines for monitoring student growth. Describe the assessment, scoring procedures. Example of action steps: Description of group: 12 students receiving psychology support in a secondary setting across 3 schools. Strategies and evidence: Student- centered collaborative discussion and role play presentations and reflections. Monitoring timeline: At the end of each unit or once a month. Assessment: Percent occurrence of targeted behavior documented on the SLO observation form.

Learning Targets … Learning Targets …are set by the professional and predict the expected amount of growth students will make during the instructional learning period. Information used in setting targets: For each student: Determine a starting point (baseline) by describing what the student can do. The corresponding DAS level is 0. Identify an expected growth target (post-test) for each student and corresponding DAS level from 1-4. Example of Targets for students A-L: Students A, C, D, E, F are able to [ describe ] and are expected to move to DAS level 2. Students B, G, H, I are able to [ describe ] and are expected to move to DAS level 3. Students J, K, L are able to [ describe ] and are expected to move to DAS level 4.
