1 Housing “D” Proposal

2 D. and her family, along with Partners for Planning, have developed a detailed plan for her to live in a self-contained apartment in her family home for the long term. The plan is a partnership between support workers, colleges, agencies, family members and government to provide the necessary supports for her to live in her own home and to actively participate in her community.

3 D is a young woman who, despite her complex developmental, physical and significant medical needs has been a participating person in her community, attending her neighbourhood school, actively involved in her church and other community groups. She wants to share her gifts and passions with support from people who understand the importance of her involvement as a participating citizen in her community – and who also have the willingness and knowledge to support her complex medical needs.

4 D has a large extended family whose cultural values are such that they are willing to learn how to assist and participate in D’s care. Her three adult siblings are living in the family home which is their cultural norm. The plan is that when the parents are no longer in the home, D will live with her siblings. D‘s mother’s knowledge will be used to develop tools and help train support workers, family and community members who will assist and provide care for D.

5 Key Elements of Proposal Family to Modify Family Home Develop a Personal Support Network Develop Training & Communication Templates P4P will facilitate the creation and ongoing support of D.‘s support network. Tools will be created and developed for knowledge transfer among family members and new supporters, including tools to track D.’s care.

6 Key Elements of Proposal Involve Extended Family in D’s Care Collaborate with DSW Programs to Create a Pool of Support Workers with Preliminary Health Needs Training Ongoing Family Support and Education P4P’s Resource Network will help to train and mentor new supporters. Learnings from this initiative will be shared on the P4P Resource Network so that others may gain knowledge and insight from D.’s experience.

7 Develop affordable viable strategies that allow people with complex medical needs to live a full life and receive medical support How do we maximize the collaboration with all stakeholders in the community to ensure that all people are supported to live the lives they desire, fully engaged in their community regardless of their level of need? 12 Long Term Impact Develop collaborative strategies with DSW educational programs For individuals who require nursing care and also require support to be fully in the community contributing their gifts, how do we find, train and mentor supporters who are good at both?

13 Long Term Impact Explore creative strategies of capturing and sharing the essence of D’s personal support needs

14 Long Term Impact Learn how to support family caregivers in "holding on differently"

15 Long Term Impact Document the learnings and share on the P4P Resource Network www.partnersforplanning.ca
