Some Barriers For Injury Prevention From Unknown Hazards Barriers to Protect Against Unknown Hazards Re-doing accelerator safety analyses every 5 years Re-doing JRAs and FRAs every three years Performing documented work planning (procedures, permits, safety committee reviews) Obtaining feedback from workers (surveys, WOSH Committee, discussions during work observations) Worker planned work (from SBMS SA): - From Step 3 + …? + Are conditions appropriate for work to proceed? + If at any time the worker is unaware of answers to the above questions he/she should suspend work and contact his/her supervisor or ESH Coordinator for assistance - From Step 4 + If the authorized work is not proceeding as planned, or new hazards are introduced or identified, the worker must re-evaluate the work or initiate Stop Work Authority if imminent danger exists 2
