2. It is mandatory for all grade 12 students in the province of BC Upon completion of ALL FOUR COMPONENTS, students will earn an RM (Requirements Met) It is worth four credits towards graduation

3. Graduation Transitions There are four required parts: 1. Community Connections 2. Personal Health and DPA 3. Career and Life 4. Presentation All four parts must be completed.

6. 1. Community Connections 1. Document at least 30 hours of work experience and/or community service. 2. Complete the response. 3. Record this on your CHECKLIST

7. 2. Personal Health 1 Document 150 minutes a week of moderate to vigorous physical activity ALL YEAR LONG! DPA on portal 2 Complete a response* 3 Develop a long-term personal healthy living plan by completing: 1 Positive Healthy Choices activity* and response* 4.Complete the Emotional Health Management activity and response * Record these on your CHECKLIST.

8. 3. Career and Life 1. Complete the Career and Life Plan – this must include a resume and a cover letter - UPDATED 2. Complete the Emotional Health Management activity and response 3. Drop off in the library at the front desk.
