Why am I here … Joined the hospital team in 1995 Now have 33 years experience of NHS Have seen various IT systems come ……and go ! Always had a feeling that we can do better and safer for our patients Hold a commercial pilot qualification Aware of the enviable safety record of the aviation industry Have undergone human factors/team resource management training

Two examples…. Probably never been to hospital One visit to Emergency Department One visit to Fracture Clinic That’s it Multiple previous visits to hospital Very likely to have many different medical problems - interacting Very likely to need help from different teams – in and out of hospital
