4 SAN DIEGO COUNTY’S ADRC COMMUNITY & SERVICE AREA ₪Serve all of San Diego County ₪Serve people of all ages, abilities and incomes ₪Population of 3+ million ₪Aging & Independence Services (AIS) & Access to Independence have satellite offices located throughout the County

5 SAN DIEGO COUNTY’S ADRC ADVISORY COMMITTEE ₪Formed in 2010 to provide guidance regarding the operations of the ADRC, recommended and adopt program strategies to enhance and sustain the ADRC and address program and policy issues related to the ADRC on a quarterly basis. DRC

7 CORE SERVICES ₪AIS’ Call Center receives:  Approximately 4,900calls/month  Around 200 web referrals/month  Intake for more than 30 programs ˗ Call Center is supported with OAA Title IIIB funding, IHSS and APS ₪Access to Independence  Receives approximately 1,500 calls/month  Funded by the Department of Rehabilitation ‡ Enhanced Information & Assistance I&A/R

8 CORE SERVICES ₪San Diego Veterans Independence Services at any Age (SD-VISA) is a partnership between AIS and the San Diego Veterans Administration (VA) Medical Center  VA has saved $1.6 million  415% increased hours of care  100% report improved QOL ₪Care Enhancement ₪Peer Counseling – Access to Independence provides one-on-one or support group setting counseling to persons living with disabilities as well as families and friends ‡ Options Counseling

9 CORE SERVICES ₪AIS case management programs, SOAR (OAA Title IIIB funded) and Linkages, provide short term service coordination to consumers ages 18 and over ₪AIS has contracts with 13 hospitals for Care Enhancement ₪AIS has contracts with Health Plans for MSSP-Like services ₪Access to Independence also contracts with a managed care health plan to provide service coordination and transitions support ‡ Short-Term Service Coordination

10 CORE SERVICES ‡ Transitions Services ₪AIS established contracts with four major Health Systems to provide CMS’ Community- based Care Transitions Program (CCTP) serving over 40,000 high-risk, fee-for-service (FFS) Medicare patients to date ₪Access to Independence provides transition services to people residing in SNFs that is funded through the Dept. of Rehabilitation, donations and grants In-Home Services Day Health Care Acute Hospital Transit Skilled Nursing Facility Medical Specialty Medical Specialty Meals Service Primary Care
