2. Project Objectives Collect information about support groups, alternative therapies & community activities available in the middle TN area for cancer patients and families. Create easily read charts patients can organize important medical information (doctors, tests, treatments, meds, etc.) Organize and assemble notebook so that clients can place and access all medical and support information in one place

3. Organized Personal Medical Record Contains list for patients to write their doctors, specialists, therapists, etc. and their contact information. Includes table of patients meds- what it is, when they need to take it, how they should administer and who prescribed it. Holds information about general cancer support groups in the middle TN area.
