2 An Incident (or Near Miss) is: “any unexpected or unintended event … that leads to (or could have led to) harm, loss or damage to people, property or reputation” RCHT Incident Reporting and Management Policy and Procedure June 2014

3 If you witness an Incident / Near Miss Ensure the immediate health needs of individual(s) affected have been attended to Ensure the area has been made safe and that potential hazards are removed Tell your line manager as soon as possible Line managers will take any further immediate actions necessary following the incident Medical Physics should be notified of any incident that relates directly to suspect/defective medical devices or equipment. The duty pharmacist should be notified of any incidents relating to defective medicines or adverse drug reactions.

4 Report the Incident / Near Miss This is NOT OPTIONAL. We cannot improve patient safety without reliable information Report as soon as possible Datix is the database used for recording the incident and the outcome of the investigation, enabling analysis by themes/ trends/ location/ causes Datix is not an incident or risk management tool

13 Basic Don’ts Do not log in to Datix before reporting an incident - if you do, notification emails will not be generated Do not enter person identifiable information in the main text sections of the incident report Do not record your opinion about what happened - just the facts as you witnessed them at the time Do not use UPPERCASE – there’s no need to shout!!
