2 PROVISION OF SUPPORT SERVICES FOCAL POINTS Home support is separate from housing and that additional services/supports that are not included in the Program Participation Points are subject to corresponding fees

3 Financial requirements for OLFI Regular Members (Service- Recipients): A one-time membership fee in the amount of P 180,400.00 shall be required from Regular Members. The membership fee entitles the members to the following:

4 A. Lifetime Membership to OLFI B. Use of “My Big House” amenities – all year-round Social Hall Use (Great Room) Children’s Room Use Teens’ Room Use Library/Art Studio Use Concierge Services

5 To ensure active and regular participation in OneLife’s community programs, a Community Fee is charged for each ISN member. This will entitle the ISN’s participation to the following services with an equivalent of 180 points per annum (equivalent of P 9,000.00/annum).

6 C. Program Participation Points: Psychoeducational Assessment/Career Preference Testing/Consultation/Counseling Monthly Community Meals Participation in all scheduled community activities ( we aim to schedule at least 10 activities per week)

7 D. Additional Program Points as Perks to Members who avail of Option 2:5 PLUS Option 2:5 PLUS (5-year program) with Perks Lifetime Membership = P 180,400.00 PPP = P 44,757.61 Upkeep Fee = P 113,385.95 TOTAL = P 338,543.56

8 D. Additional Program Points as Perks to Members who avail of Options2:10 PLUS Option 2:10 PLUS (10-year program) with Perks Lifetime Membership = P 180,400.00 PPP = P 91,000.00 Upkeep Fee = P 229,420.75 TOTAL = P 500,820.75

9 Perks PROGRAMDESCRIPTION Options in Using Points VALUE/annum Respite CareBoard, Lodging and Respite Care for (1) ISN or 1 Elderly for 7 days/year; bundled with Guest Room Use 1. Exchange Program Participation Points with any other services offered on- site 2. Donate it 3. Sell it 4. Give it as a gift (Note) Options should be transacted with families with ISN/Elderly members only and shall be facilitated through OLFI P 14,000.00 or 280 points

10 E. Seniority in Housing Queue – an OLFI Regular Member does not have to be interested in co-owning a housing unit, however, he shall have the right of seniority in the queuing for housing once he has completed the payment. Interested renters will also come from this queuing list.

Teaching Kitchenfor preparing community meals/ On Enrollment basis if to be used for the ISN’s Vocational Program /for Events
