1 (PPW) PERSONAL PLANS FOR WELLNESS

2 A COUPLE MINUTES Take 2 mins and jot down all of the things that you do for your personal wellness as of right now. Take 1 min to include all the things you would LIKE to do for your personal plan.

3 PURPOSE The primary purpose for WELLNESS 10 is to FACILITATE OPPORTUNITIES FOR STUDENTS TO TAKE RESONPSIBIBLITY FOR MONITORING, ENHANCING, AND EVALUATING THEIR OWN WELLNESS.

4 YOUR ROLE independent It is important for you as high school students to become more independent and take charge of your wellness outside of the school environment.

5 PPW demonstrate responsibility entire course You, as students, will demonstrate increasing responsibility for your own wellness by designing, implementing and evaluating a comprehensive Personal Plan for Wellness (PPW) for the entire course (until Jan).

6 ANSWER THE FOLLOWING QUESTIONS What is a balance and happy life? What is human nature? How does it influence the well-being of yourself and other? How and why do particular inequities (both perceived and real) impact the well-being of communities? How do your doubts and fears affect the balance of your life?

7 REFLECT ON THE FOLLOWING… What matters the most to you in a relationship? How does being physically fit enhance your overall well-being? How might your confidence and competence in your physical abilities impact your overall well- being? Why and when is it important to step in or speak out in someone’s defense? Why do people sometimes hurt others who have done nothing to them? What role do ethics play in balanced wellness?

8 GETTING STARTED Look at your Wellness 10 inventory that was handed out to you at the beginning of September. Which dimension is the LOWEST? Which dimension is giving you the “flat tire”?

9 WELLNESS 10 PPW “WHAT” BREAKDOWN Instructions Instructions: How you create your wellness plan is up to you- there is no set format- below is a guide of things to include. HOW you include them is up to you- you complete the parts in GREEN.

10 COURSE The whole course is like an action plan. You need a goal for the whole year based on Wellness Wheel Assignment Each outcome is a part that helps you to achieve your year goal. Each outcome needs it’s own goal which helps for the year goal.

12 YEAR GOAL SETTING THE YEAR GOAL: Complete wellness wheel personal assessment and create SMART goal based on results. Ex: Goal for Year: By the start of January 2013, I would like all my wellness areas to have increased to a 4 based on the Wellness Wheel Assessment. 13 OUTCOME GOALS In order to set a goal you need to understand what the outcome is. To start, read the points under each outcome- it will give you details as to what it is asking. If you can do the little parts underneath, you can do the outcome!

14 UNDERSTANDING OUTCOMES Complete the following table to test if you understand what each outcome is asking. If you check YES in the Understanding column, skip to creating goals chart an complete the appropriate goal. If you check NO- complete the rest of the row for the specific outcome.
