5 Medical Plan Changes In 2014 you have the choice between two new medical plan options - the Core Plan and the Enhanced Plan. These plans allow YOU to choose the out-of-pocket expense, per-paycheck contributions and level of coverage that is right for you. Both Plans include Delhaize America contributions to a CDHP Fund to help reduce your annual deductible and out-of-pocket costs. Your medical carrier will depend on the state in which you live: Aetna is the medical carrier for all states, EXCEPT North Carolina and South Carolina. BCBSNC is the medical carrier for North Carolina and South Carolina.

6 Medical Plan Changes Here is an overview of the two medical plan options; more details and a comprehensive comparison chart are included in your 2014 Benefits Guide, available at myDZAbenefits.com. SimilaritiesDifferences Both plans: Cover the same eligible expenses Cover care in- and out-of-network, with greater benefits when you use network providers Pay 100% of the cost of in-network preventive care Protect you from really high costs through an out-of- pocket maximum. If your expenses reach this maximum amount, the Plan pays 100% of eligible expenses for the rest of the year. Cover prescription drugs the same way, with the same copays Have the same eligibility requirements to earn Premium Credits in 2014 The plans vary by: Delhaize America contributions to your CDHP Fund Per-paycheck contributions are lower for the Core Plan compared to the Enhanced Plan Coinsurance amounts are lower in the Enhanced Plan Deductible amounts are lower in the Enhanced Plan compared to the Core Plan Out-of-pocket maximum amounts are lower in the Enhanced Plan compared to the Core Plan

7 Medical Plan Changes Your out-of-pocket costs will depend on the choices you make when you need care –In-network specialists and hospitals will be grouped into two categories, called tiers: Tier I Providers of Distinction – Your out-of-pocket costs will be the lowest when you select a high quality & efficient provider (specialist or hospital) from this tier Tier II* – You can choose a Tier II provider (specialist or hospital), but your out-of- pocket costs will be higher *Associates eligible for BCBSNC in Fayetteville, North Carolina or South Carolina will be covered at the Tier I benefit level for eligible services provided by in-network providers. Tier II benefits are not available in these areas.

8 Castlight With Castlight, you can: –Compare in-network doctors, facilities, and medical services based on quality, cost, and convenience. –See personalized cost estimates based on your specific health plan, the rates that have been negotiated with your insurance company, and whether or not you've met your annual deductible. –Review step-by-step explanations of past medical spending so you know how much you've paid and why. –Receive recommendations about ways to save money and find high- quality care.
