Miscommunications are: Common Hazardous A leading cause of adverse events in hospitals A contributing cause in 2 out of 3 sentinel events

Why not SBAR? SBAR is a communication tool designed for escalation of care – not designed for handoff We will continue to use SBAR for escalation

Why IPASS? IPASS “I pass” the care to you
Communication tool designed for handoff Provides a structure to deliver a succinct & efficient report Used by the Medical Staff at CNHS & shown to improve the quality of their handoffs Incorporates a “shared mental model” into the report process “I pass” the care to you

The Elements of IPASS Introduction/Illness Severity Patient Summary
Who is this? Name, DOB, MRN, Age, Weight, Allergies Why are they here? Diagnosis, Severity, On Admission – what team they are going to Patient Summary Past Medical History Events Leading to Now Review of Systems (head to toe assessment summary) Access/Pain score Action List What needs to happen (Plan of care, Labs, Tests, Procedures, Tasks) Situational Awareness & Contingency Plan Family or Social Needs Language Clinical Concerns Discharge Planning Summation by the Receiver Receiver summarizes what was heard Receiver asks questions Handing off RN verifies information transmitted was accurate

Provide a brief summary of the report you heard
What Do We Need To Do GIVING REPORT Gather information & ASSESS the patient if you have not already Organize information into IPASS format Open the chart Provide report using IPASS Ask for the receiver to provide a summary of report Listen to receiver’s summary & CORRECT any misconceptions RECEIVING REPORT Listen to report Minimize interruptions – ask questions at the end, not in the middle Provide a brief summary of the report you heard
