1 PAPERWORK GUIDELINES Getting Started

2  Placement Paperwork – This is what is needed for a placement to be considered complete.  - CBC  Must have a Criminal Background Check (CBC) completed by each member of the household 18 years of age and older (this includes college aged children who will come home on breaks, elderly family members, friends/roommates, etc.)  Host Family Application  7 page application form Must be signed and dated by the Host Family  References cannot be family members or the local Rep  You should call the references provided by the Host Family, fill out the Reference Form, and sign and date it certifying that you did call and interview the reference.  You may place a student on hold for 3 day to complete the above steps.  Once the CBC’s have cleared you will have 1 week to complete the placement process including High School acceptance.  We will not continue to hold the student if you do not meet the this timeline.

3  Home Interview  9 pages including the Host Family Agreement, additional information form, Acknowledgment of Receipt “The Exchange Visitor Program”, High School Acceptance Form and Host Family Recommendation Forms  You should take the required photos while doing the Home Interview. If the family has taken the photos, verify that they are the right photos and have all the needed components (i.e. stove and refrigerator for the kitchen, etc.).  Photos  There are 6 photos that are required by the Department of State, these include:  Family photo – must include everyone who lives in the home  Front of the house – must include the entire house and front yard and back yard.  Student’s bedroom – must clearly show the student’s bed and at least some storage space –  the bed cannot be an inflatable mattress, single mattress on the floor, pullout couch, or any other type of convertible bed  Student’s bathroom – must include the toilet, sink, and shower/tub Wide shot please.  Living room or family room Kitchen – must include the stove and refrigerator  Please make sure that the house looks neat when taking the photos–this is the student’s first impression of where they will be living  You can always include more photos!  School Agreement  Must be signed by the person responsible for admitting exchange students to that school (Principal, Superintendent, etc.)  Please make sure that the school information is filled out completely and that the exact dates that school starts and ends are included (Please remind the school to notify you if dates change.)  Once the national office has all the above forms, this is a completed placement and the office will submit the placement to the overseas agent, student and natural family.  You will be paid once all has been submitted 10 month student placement fee of $425 and 5 month placement fee of $275  Please allow a two week turnaround from the time the placement is competed for payment.

4  Double Placement Agreement  This is needed if you are placing two students with the same family The family must sign agreeing to host both students The students cannot be from the same country or speak the same native language  The office will get the DOS required signatures from the student’s natural parents.  Third Reference for a Single Adult without Children in the Home  The Third Reference is only needed if there is one Host Parent without children living in the home.  Cross Program Placement Agreement  This is needed if you are placing a student with a Host Family that is already hosting/planning to host a student from another Student Exchange Organization  The form must be completed and returned to the office so that it can be sent on to the other organization Supplemental Placement Paperwork – These are only

5  What happens after the student is placed?  Arrival Orientation  To be completed with each Host Family and student. This is a time to review the WISE and Department of State Rules and Regulations and to go over any last minute questions the families or students may have (health insurance, house rules, who will take the student to school to get the student registered, make sure the family has all your contact information, etc.)  This is a time to go over the WISE rules and regulations with the students and host families and you must handout and go over information on sexual exploitation, abuse, and neglect  Individual time should be spent with each student so that this can count as your first in-person visit with the student and host family. Review host family and student handbooks.  This is needed even if the student is moved to a new family at any time during the placement.  Orientation form must be signed and dated by the students, host families local representative  You will be paid once all orientation forms have been submitted, $125 for either 10 and 5 month students. Please allow a two week turnaround for payment.

6  2 nd Visit  This should be completed by someone other than the person who placed the student.  It does not have to be a Local Representative–it can be the friend or coworker of the Local Representative or Host Family but it cannot be a relative of the Host Family or the Local Representative The person doing the follow-up visit must complete a one page form for each exchange student in the home - must be within 30 days for a Welcome Family and 30-60 days for a Permanent Family.  The person doing the follow-up visit will be paid $50 per home not per student in the home

7  Monthly Supervision  This is a one page report to be completed monthly You must see the student in person or you must speak to them by phone. We will not accept electronic communication.  You must speak with the student, the host family, and someone at the school monthly. You must see the host family once in person each semester.  Be honest–identify any problems so that we can get help from the Agent/Natural Parents in correcting the problem. Be thorough–we need sentences, not just a few words.(Remember–if it was your child, you would want to know more, so give at least two or three sentences. If not more!)  You will be paid monthly $55 per student for 10 month students and $60 for 5 month students.  Your first monthly supervision report starts in September and is due Oct 1st  Monthly Supervision is due on the 1 st of the following month.  Payments will be mailed no later that the 15 th of the month  If for any reason WISE is required to fulfill the paperwork requirements by completing it directly from the student or the host family there will be a $30 reduction in payment per form done by the National office.

9  Additional Paperwork  Travel Release Form To be completed if the student is traveling for more than two nights with their Host Family or for any period of time with someone other than their Host Family  Should be submitted as soon as the trip is arranged–the office needs to submit these to the student’s agency to have the student’s natural parents sign giving permission.   Counseling Report  To be completed anytime a counseling session is needed. Should include documentation of the issues and recommended problem resolution. Please seek your Regional Director for further assistance in the conflict resolution.  Warning and Probation Letter  All Warning and Probations are done with counseling reports and communication between the LR, RD and the national office. The national office will prepared and write the probation letter. The Probation of a student will be handled from the national office, with consideration of the counseling reports and documentation of the local Rep. WE ARE NEVER TO TELL A STUDENT WE WILL SEND THEM HOME! Dave Dahl is the only person who can make that determination.

(This includes contact with the student, no matter how minor the issue/concern is.) 12 Please advise the natural parents of the travel plans of the student listed below and confirm the natural parents’ permission for the travel. This form is required by the WISE National Office at least 3 weeks in advance of ALL student travel for more than two nights. This form is not permission to travel unless it is signed by the AYA Manager or President in the national office. Adequate time should be allowed so all parties can sign the form. This form needs to be given to your local representative. The form will be forwarded to the national office for final approval. Please print clearly except where signatures are required. By signing the form all parties are stating their agreement with the above travel arrangements. Permission is only given when all parties have signed. If for some reason travel is outside the U. S. and proper approval from all parties is given, WISE cannot guarantee to re- entry of the student back into the U. S.
