3 Introduction This application is used to build dynamic and engaging forms of different type like surveys, polls, ratings etc… It provides professional and casual users with a way to ask website visitors and customers’ direct questions complete with customizable interfaces and response stats. It can be used for different purposes.

9 User account Your Account: After verifying your account through your email id, you will get an access to your account. As an administrator of your account you have all rights on following modules: 1. Account Settings 2.Team Management 3.User Management 4.Dashboard

10 User account 1. Account setting: 1. time zone 2. Default currency 3. Date/Time formats Account Summary 1. Your subscription: contains information of your account subscription plan 2. Contact information: Maintain your/company’s profile

11 User account Team Management Multiple users can manage the forms that you create. Team management module allows you to add multiple teams containing users with their roles. As an administrator you can manage your teams, users and assign roles to each user in team. Team Users Roles

12 User account User Management Maintain your end users in a list/group. Group Users: name, email address, group, Note: Can import and export users as CSV, XLS file. GroupUsers have many

13 User account Dashboard / home: It will contain the list of your existing/created forms. Manage/organize your forms in folders. Following features will be available on dashboard. 1. Existing forms list 2. create new folder, copy forms into folder, delete existing folder. 3. Create new form.

16 1. Build Your Form 1. Create form: user can create a new form or select the existing project from the list. Following options will be provided: 1. Build from scratch 2. Copy existing (drop down having existing forms ) 3. Use template (categorized predefined forms )

19 1. Build Your Form Welcome page: Customize your welcome page as you want: 1. Form title 2. write welcome text. 3. Your business logo

20 1. Build Your Form Content page: Organize your questionnaire in pages and sections. Form can consist of multiple pages. Inside the page you can define multiple sections (its optional). Section can exceed from one page. Thank you page : Responses are marked as complete when they reach this page. Add thank you text in the end of the form.

21 1. Build Your Form 2. Prepare questionnaire: you can define your own custom question using defined question type or you can choose from question bank. Question type: 1. Single choice (radio buttons) 2. Multiple choice (checkboxes) 3. Text 4. Classification (dependent dropdown) 5. Email 6. Date 7. Slider 8. Star rating grid 9. Essay (long text) 10. File upload Question bank: select predefined questions from the question bank. Questions were defined with answers in respective categories.

22 1. Build Your Form 3. Question actions: Following actions can be performed on each question: 1. Edit 2. Copy 3. Re-arrange 4. Delete 4. Question Logic: 1. hide the question after answering 2. Disable question 3. Randomize logic (don’t change position, always include in page randomization)

23 2. Design your form Designing includes the coloring, adding theme or customizing theme using available palette. By customizing themes refers to changing of the colors or text font in existing theme. 1. Pre built Themes (Only existing themes can be applied.) 2. Color & images: Available colors can be used to color the button, background and text Color palette (color combination to design over all form color) Background (Available background images for setting a background will be available also user can add images for the background too) 3. Fonts (Text style can be changed using the available fonts)

24 3. Publish your form Once the form is built and designed, the form can be published / distributed via various ways. 25 4. Analyze your result After getting response you can analyze the result of your created form. Result visualization is categorized in the following way: 1. Overall response summary It display graphs on 1. no of responses Vs. date of response 2. no of response Vs. response device 3. response rate Vs. response status 4. Response rate Vs. Location 2. Individual response 3. Question level response 4. Custom reports (on selected questions) Note: you can share your result, export it into csv, xls, spss, pdf and.txt formats.
