Ryder Trauma Center Documentation Issues What constitutes a good “patient problem” description? What is the best way to document this information? How does technology for documentation (e.g., Cerner system) impact the ability to capture good patient descriptions? What strategies do nurses use when relying on technology for documenting patient information and how can these strategies translate into human error?

Ryder Trauma Center Incident Reporting Systems in Healthcare These were motivated by the Aviation Safety Reporting System JHS invested in one such system about 5-7 years ago Issues related to training nurses on use of such systems –(e.g., do they understand what an “incident” is?) Issues related to extracting and effectively utilizing these data –Who analyzes these data? –How are these data analyzed? –How are the results of these analyses fed back to medical providers?

Patient Safety Center at JHS How can shift changes (“handoffs”) translate into human error and adverse outcomes? How are technologies being used in handoffs, and what are the risks associated with such use? What training strategies might be implemented to teach providers about the risks associated with handoffs?
