5 Improving the management of Hypertension 2004/5 Mortality rates- Premature mortality from hypertension in Dudley was nearly double the England average Variation in expected to actual prevalence of hypertension Inter-practice variation within CCG Action:- clinical audit, use of PBPs to review, diagnose and manage patients

6 The evidence: Prevalence of hypertension has increased since 2004/5 from 13.4% to 17.7% in 2014/15 2014- >2000 patients reviewed, 1096 diagnosed For every 1,000 patients controlled, 16 strokes and 12 MI’s could be prevented each year
