3 3 Key Strategies and Initiatives Effective patient centered primary care transformation efforts across the age spectrum, leadership development, and culture change: -team based care -focus on high and rising risk -leverage technology -expand primary care based services including integrated BH and community health teams -continuous quality improvement

4 4 CTC Considerations  Goals include sustainable primary care transformation that supports improved health, better experience of care, lower per capita cost, AND improved work environment for primary care (bring “joy” back into the work)  We have been least effective in: “person and family engagement” and “bringing joy” back into the work of primary care.  With increased funding has come increased accountability with the tension of how to measure and how to assure that accountability  We are still in the “harder part” of transformation where the increased burdens have not yet led to all the benefits of change…but we are getting closer to that “tipping point”  Last meeting included a focus on larger organization innovations to identify and engage high risk patients/ transitions of care. Today a focus on OHIC standards and a powerful practice-based tool that could work for all types of practices.

5 Questions  What is the best way to recruit smaller practices to engage in primary care transformation?  What is the best way to make these quarterly meetings in the most effective ways? 5
