Hoyle Pond Underground – UPC Shifter Lost Time Accident November 25 th, 2010

2 Accident Description Supervisor driving a UPC Shifter #007 is traveling down the main ramp from 1020 ml toward 1140ml At the remuck approx. 80 feet before 1140ml, it is suspected that the UPC contacts the right wall (at the remuck corner) and flips over onto the drivers side. The supervisor is pinned under the roll bar of the UPC shifter. A worker arrives approx. 5-10 minutes later, gets help from a Dumas worker and they flip the UPC off the supervisor. First aid is performed on the supervisor and the supervisor is transported to surface in the 720 Emergency Vehicle. 2

13 Accident Description Following the accident the UPC shifter was tested by the Ministry of Labour and Goldcorp maintenance personnel. The unit was found to be in good operating condition, no defects detected with the unit including steering and brakes. 13
