3 The Quality Challenge Medication Reconciliation is the gold standard for reducing medication errors. Adopted by Accreditation Canada as a required organization practice. Currently no standard medication history, medication histories incomplete, no standard medication communication with our community partners, families frequently leave clinic without a written medication history.

4 Background A reviewed of clinic worksheets, nursing history forms to assess how medication histories were completed. Six high risk clinics were initially selected for trialing.

5 Intervention phase 1) One standard med rec form for all clinics. 2) Other forms of medication history removed. 3) Face to face audits based on Accreditation tests of compliance. 4) Workplans developed.

6 Effects of change Nursing medication history standardized across the ambulatory care setting. Nurses and physicians are able to provide focused medication education to client’s and families.

7 Role of the Family Families and clients are more engaged in their medication history. Families and clients are provided with a written record of a best possible medication history.
