1 Required Views: Single tooth and Edentulous segment of two or more adjacent teeth All photographs must comply with applicable patient privacy laws Do not submit any photographs that are not required. 1) Centric occlusion showing the restored tooth or teeth and the embrasures and gingival architecture, including free gingival margins, papillae, and surrounding attached tissue and mucosa. Make a close-up facial view showing no more than three full teeth in one photo. A span of four or more teeth will require at least two photographs. 2) Right lateral working movement that shows the restored tooth or teeth. 3) Left lateral working movement that shows the restored tooth or teeth. 4) Protrusive working movement that shows the restored tooth or teeth. 2 Required Views for: Edentulous Cases 1) Frontal (maxillary and mandibular teeth in occlusion) 2) Protrusive 3) Occlusal view of the arch that was restored For cases that include a removable prosthesis, three additional photographs are required: All photographs must comply with applicable patient privacy laws Do not submit any photographs that are not required. 4) Lateral view in centric occlusion, left 5) Lateral view in centric occlusion, right 6) Lateral view of left working 7) Lateral view of right working 8) An occlusal view of the superstructure without the removable prosthesis in place. 9) A frontal view of the superstructure without the removable prosthesis in place. 10) A view of the intaglio (tissue side) surface of the removable prosthesis. 4 Required Views for Graft Cases 1) Pre-surgical occlusal view showing the atrophic ridge 2) Pre-surgical facial (lateral) view showing the atrophic ridge 3) Immediate post-surgical occlusal view 4) Immediate post-surgical facial (lateral) view 5) Occlusal view of healed site, typically 4 - 6 months after ridge augmentation and pre-implant placement 6) Facial (lateral) view of healed site, typically 4 - 6 months after ridge augmentation and pre-implant placement All photographs must comply with applicable patient privacy laws Do not submit any photographs that are not required.
