What is it? Brucellosis is also called MALTA FEVER,it’s a zoonotic disease caused by brucella organism which enter the body through MOUTH,RESPIRATORY TRACT,GENITAL TRACT OR ABRADR SKIN. HOW DOES IT SPREAD?? >>>spread is usually from from ingestion of RAW MILK FROM INFECTED CATTLE OR GOATS. >>the Bacilli usually travel in the lymphatics and infect the L.N FOLLOWED BY haematogenous spread with localization in the RETICULOENDOTHELIAL SYSTEM . >>>INCUBATION PERIOD 1-3 WEEKS.

CLINICAL FEATURES ONSET..
ACUTE>>with high continuous fever OR INSIDIOUS>>with fever undulating over 7-10 days . SYMPTOMS.. Fever,sweating,weakness,headache,anorexia,PAIN IN LIMBS AND BACK,rigors,joint pains. SIGNS.. *FEVER *LYMPHADENOPATHY *HEPATOSPLENOMEGALY *SPINAL TENDERNESS

COMPLICATIONS *LOCALISED DISEASE..
Causing suppurative or granulomatous lesons including>>ARTHRITIS,SPONDYLITIS,BURSITIS,OSTEOMYELITITS,MENINGOENCEPHALITITS, PNEUMONIA,HEPATITIS. *CHRONIC BRUCELLOSIS.. LOW GRADE FEVER & NEUROPSYCHIATRIC SYMPTOMS.

INVESTIGATONS *FOUR –FOLD OR GREATER RISE IN BRUCELLA ANTIBODY TITER OVER A PERIOD OF 4 WEEKS IS HIGHLY SUGGESTIVE OF BRUCELLOSIS. *BLOOD&BONE MARROW C/S ARE POSITIVE IN 50% PATIENTS IN ACUTE ILLNESS.

MANAGMENT *COMBINATION OF >>>DOXYCYCLINE 200mg daily + Rifampicin mg daily FOR 6 weeks.
