3 Lake County Free Clinics mission is to serve the community of Lake County who are uninsured or under-insured and in need of compassionate health care at no cost. The clinic will create a supportive environment where the focus is promoting healthy living through prevention, treatment and education.

6 To provide excellent health care to the uninsured of Lake County. Each patient is a unique individual with unique health care needs. In response to those needs, care will be tailored to each person. Every patient can expect moral and compassionate care delivered in an environment that is respectful to race, culture, and ability to pay. Each patient has a right -not a privilege- to health care regardless of ability to pay. Volunteer staff will conduct care with the utmost professionalism by maintaining credentials, license, and keeping up to date with evidence based practice. 7 A dashboard to determine if policies and services offered are supporting Lake County Free Clinic’s philosophy. Develop patient services with emphasis not only on treatment but wellness. Build healthy families through education, prevention, and treatment. Recruit motivated knowledgeable, skilled volunteers to create a dynamic workforce. Promote and market out services in the community by strengthening relationships through service. Obtain solid financial assistance in the form of grants or free monies to establish a solid financial base for operations.

10 Adheres to therapeutic standards Measures health outcomes against patient centered goals Follows nursing process and standards of care Assesses the needs of individuals, families Provides care to patients with acute and chronic health problems

12 Dispenses medications as appropriate Monitors all orders for clarity and appropriateness Participates in creating therapeutic medication treatment regimens Provides education and counseling to patients regarding medications

13 1. Market available volunteer and hired positions. 2. Conduct pre-employment screening including but not limited to interviews and background screening. 3. Ensure that all staff is up to date with their state licenses. 4. Create schedules for staff and make adjustments when needed. 5. Provide staff with description of their assignment 6. Keep track of staff’s yearly TB test and CPR certifications

14 Assist patients with finding resources in the community that could be beneficial to them. Perform psychosocial assessments and interviews on patients.

15 Medical Records Clerk Responsible for Initiating and maintaining patient files and statistics; responds to requests for medical records; performs clerical duties. Receptionist The receptionist is responsible for assisting the entire clinic by answering the telephone professional and pleasant. The receptionist should be able to perform basic clerical duties needed in a medical environment. Grant Writer The Grant Writer for this clinic is expected to be able to locate private and public resources to fund operations at the clinic. Medical Assistant The Medical Assistant is responsible for assisting other healthcare staff in providing direct care with the understanding of the assessment process of nursing.
