3 Buprenorphine/Naloxone Brand names Suboxone ® Zubsolv ® Indication Opioid Use Disorders

4 Training Requirement Staff must be MAP Certified and Trained specific to Buprenorphine/naloxone therapy management Prior to administering

5 HCP Order Requirements Buprenorphine/Naloxone Is ordered only by a Specially trained physician ‘Waivered Provider’

6 HCP Order Requirements HCP order must specify Medication is indicated for Treatment of Opioid Use Disorder or Similar terminology

7 Buprenorphine/Naloxone If prescribed for pain management It generally cannot be administered By Certified staff Except as last resort as indicated on Healthcare Provider letterhead Submitted to DPH Drug Control Program

8 Prescription Requirement Buprenorphine/Naloxone Can only be prescribed by a Specially trained physician ‘Waivered Provider’

9 Buprenorphine/Naloxone Countable medication Schedule III Tamper resistant packaging Double locked Reconciled when keys change hands

18 Single Entity Buprenorphine Drug Products Used for acute detoxification Only administered in a Treatment facility Never in a MAP setting
