3 3 Why Change Current System of Care? Current system is not outcomes-based and does not incentivize permanency. System is not integrated which creates substantial barriers to permanency. Loose contracting processes have resulted in an over-abundance of high end services without the necessary referrals to sustain them. Home and community-based work has not been well funded and a significant service gap exists makes sustainable permanency difficult. Current system does not lend itself to easily fulfilling CFSR or Kenny A. requirements.

5 5 The Office of Provider Utilization and Outcome Management (OPUOM) OPUOM will be comprised of 4 Units –Network Development –Contract Monitoring –Technical Assistance –Home-Based Services OPUOM will be focused on the development and management of a comprehensive, community-based, coordinated network of provider services that reflects our values and principles. Purchased services will be targeted, measured and developed with permanency in mind. Provider partnerships are a key to the success of children and should be cultivated through honest collaboration.

7 7 Key Points Shared responsibility is the key to accomplishing our goals. Innovation is the pathway to permanency for our most troubled youth. We must measure and hold each other accountable. Accomplishment should be recognized and rewarded.
