3 LTCIP Strategies Reviewed n Managed FFS model: –MD focus group: May 19 –CMS Chronic Care Improvement Initiative n Fully integrated model: –AAP review; consensus June 9 PC meeting –Dr. Meiners, legislative committees and –SB 1671 allows for MassSCO replication –Budget bill proposes $.5M: Contra Costa & SD

Network of Care Strategy n AoA Aging and Disability Resource Center grant to California (2 counties) –Analyze current programs/processes –Identify modifications to enhance –Partner w/MDs, other professionals –Conduct public awareness activities –Target underserved, hard-to-reach

San Diego’s Proposed Role n Fund NOC Strategy n Approximately $500,000 n Over 3 year period n Starts July 1, 2004 n Resources for testing, enhancing NOC n Support local strategies/objectives

Network of Care/Trilogy n A comprehensive website designed for older adults, people w/ disabilities, their caregivers and providers (www.networkofcare.org) n Features: –Resource finder for services, facilities & assistive devices –Consumer-focused health information –Caregiver support options: message board, chat, articles –My Record: private online file of medical/legal info; can be shared w/ others

New to Support all Strategies: Community Education Workgroup n Broad-based information warehouse w/in Network of Care (NoC) n Best practices: health & social services –Self-care management –Disease management protocols –Preventive chronic care management –Cultural competency and diversity

Goals n Develop coordinated & reliable system for I&R, assistance, assessment, education, chronic care management and communication across consumer and provider groups to: –Improve community access to a continuum of health and social services –Expand the community's ability to promote healthy behaviors and manage chronic conditions

Goals…continued –Help create new relationships in health care where consumers & professionals work better together –Develop “continuous quality improvement” program that supports all LTCIP integration strategies (NoC, Physician, Health Plan Pilots)

Strategies & Objectives n Phase I: Use NoC as early catalyst to engage/focus consumers, caregivers, providers in health education –Test & evaluate current usefulness and accuracy of website as a communication tool & community resource instrument –Make recommendations for improvements –Develop work plan for implementing necessary system improvements
