2  On November 13 th, a thermos style container used for transporting liquid nitrogen exploded in an exam room.  A clinician was in the exam room with a pediatric patient and his mother  The clinician reported that the container made a loud hissing noise. She then shielded the patient before the container exploded

5  Warning: This unit uses a glass liner that is extremely fragile. Any hairline cracks in the liner (caused by accidentally dropping, unit tipping over, etc.) can cause the container to explode. Use this container with extreme caution at all times.

6  Immediately removed thermos style containers from use (only one other found)  Ordered new stainless steel containers  Ensured PPE was available at LN transfer location  Additional training to be provided
