Improve Access to Quality Health Care and Services More than half of pregnant women and more than a third of infants and children benefit from maternal and child health block grants More than 700,000 rural Americans receive health services thanks to rural community-based grants About 115,500 parents and children participate in the national Home Visiting Program 5

Improve Access to Quality Health Care and Services Opportunities: Outreach and Network Planning Grant Programs – innovative access strategies and rural hospital planning Small Health Care Provider Quality Improvement Grant Program – managing chronic disease Frontier Community Hospital Improvement Demonstration – frontier critical access hospitals Support to Critical Access Hospitals for quality, financial and operational, and population health improvement through the Medicare Rural Hospital Flexibility Grant Program 6

Strengthen the Health Workforce National Health Service Corps 44% serving in rural communities Rural Training Track 34 programs with more than 70% of these residents continuing to practice in rural communities Workforce Programs Over 11,000 rural training sites Over 180,000 students trained in primary care, oral health, geriatrics, public health and behavioral health in rural communities 7

Build Healthy Communities 8 Coordinate health care activities for 57 million rural Americans Support providers in rural and isolated areas improve patient care with the use of telehealth, telemedicine and health IT
