Announcements, cont’d… CMS Accountable Health Communities Model Call for Grant Funding Applications – What: Over a five-year period, CMS will implement and test a three-track Accountable Health Communities Model (Track 1 Awareness, Track 2 Assistance and Track 3 Alignment) based on promising service delivery approaches. Each track features interventions of varying intensity that link beneficiaries with community services. CMS Updates its 2016 Quality Strategy – What: CMS has published an updated version of its 2014 Quality Strategy with a focus on applying the Administration’s strategy around person-centered care. Call for Participation: Help HL7 Help You Reduce C-CDA Implementation Inconsistencies – What: HL7 invites C-CDA users to complete a survey that will help clarify the inconsistencies and pain points encountered when sending and/or receiving CCDs, Discharge Summaries and/or Referral Notes. Collection of data on these three document types will continue for several months. Results will be shared via webinar once results are compiled. Colorado Department of Health Care Policy & Financing: 2015 Accomplishments 30 2015 Accomplishments Completed statewide stakeholder outreach to various groups including single entry points within DHS, Regional Care Collaborative Organizations, HCBS Providers, Community Centered Boards, The Alliance (Statewide association of Community Centered Boards) and Program Approved Service Agencies Established Focus Groups within Northern, Southern, Metro Denver, Easter Colorado and the Western Slope; conducted two focus group meetings: Spring 2015; included 255 members Fall 2015; included 210 attendees Identified target pilot population (200 clients in the two Medicaid Waiver populations of EBD and SLS IDD; confirmed multiple pilot sites per focus group area: Metro Denver/ Jefferson County/ Boulder = 11 Southern Colorado = 8 Northern Colorado = 4 Easter Colorado = 4 Western Slope = 6 Completed PHR Environmental scan with 136 individuals (clients and providers) Participated in 3 PHR Vendor Demos in Dec 2015 Identified pilot sites for Round 1 & 2 Confirmed vendor for Round 1 pilot

Georgia Department of Community Health 32 2015 Accomplishments Kicked off eLTSS pilot Identified eLTSS Pilot business drivers Identified the data flow process when a Client is on-boarding to the ICWP and CCSP waiver programs Reviewed ICWP and CCSP service plans Identified data elements and reviewed other relevant waiver program documentation to support the eLTSS pilot efforts Conducted a successful workshop (town hall) on November 20, 2015 The workshop developed a focused data profile for CCSP and ICWP waivers. Identified existing ecosystem problems HIT can help solve. The Stakeholders who attended the work shop town hall were Waiver Program Administrators, Service Providers, Case Management and AAA. Presented the outcome of the workshop to the HITECH CoP in December 2015 2016 Milestones Host two more work shop town hall sessions to continue to gather requirements to support the CMS / ONC S&I Framework initiative to develop an eLTSS plan for community-based services. Analyze the combined results from all workshops for the eLTSS pilot.

Kentucky Office of Administrative and Technology Services 33 2015 Accomplishments Collected feedback from providers and cabinet level staff on the waiver case management system for patient module enhancements. SIM Grant HIT workgroup participated in providing current status, capabilities, and the interoperability of various Health Information Systems across the Commonwealth along with a collective agreement of the need for universal standards. Used an existing relationship with University of Kentucky who also collect National Core Indicators. Prepared a vendor statement of work for WCMA PHD module enhancements 2016 Milestones Implement Experience of Care Round 2 and FASI Round 1 Implement MWMA PHD module and Round 1 eLTSS Pilot Engage consumer and caregiver feedback on Experience of Care results (1 st round), WCMA, and eLTSS record

Maryland Department of Health & Mental Hygiene 34 2015 Accomplishments Completed an Environmental Scan with our independent contractor Peter Gobielle. Mr. Gobeille did extensive research into the PHR market and compiled a thorough charter/scope document to help guide our efforts as we move forward in the project. The TEFT Team spent considerable time in the Harmonization phase, examining data domains and looking for alignments within our current, live LTSS system. Began the Work Order process with our vendor, FEi Systems, including the initial phase of requirements gathering. Hired a new TEFT Team member and Health Policy Analyst, Dan Jimenez. We are excited to have Dan on board and look forward to utilizing his skills, expanding our team, and advancing our project. 2016 Milestones Finalize eLTSS Pilot population and participant samples Complete our eLTSS Pilot Work with vendors in building out our PHR component/solution and the requirements and modifications needed for our LTSS system Prepare for our next site visit with the Lewin Group in February

Minnesota Department of Health Services 35 2015 Accomplishments Developed comprehensive RFP for PHR Community Collaborative, included detailed business requirements workbook including over 260 functional/non-functional requirements for the target PHR solution. RFP published on 04/13/2015 Conducted RFP responder's conference in April. Proposals collected in June and technical analysis and review committee meetings held in July. Conducted contract negotiations from August to October; awarded contract to Otter Tail PHR Community Collaborative in November. Collaborative will subcontract with a PHR vendor and project management consultant. Collaborative held two meetings; MN DHS staff provided detail overview of eLTSS Pilot Project. MN DHS staff conducted 1 hour information interviews with project specific staff at most of the partner agencies.
