Explain NANDA-I, NIC, NOC North American Nursing Diagnosis Association International (NANDA-I) Nursing Outcomes Classification (NOC) Nursing Interventions Classification (NIC) Are examples of standardized languages describing diagnoses interventions, and outcomes respectively that are recognized by the ANA. The groups behind NANDA-I, NOC, NIC are working together to demonstrate links between these three standardized languages &thus to improve communication both in the United States and internationally

Outcomes Identification Indicates the degree of wellness desired, expected, or possible to achieve Goal Statement(nurse) vs. Outcome Statement(patient) Guide the selection of nursing interventions Establishes the measuring standard that is used to evaluate the effectiveness of the nursing intervention

Outcome Identification Uses the word patient or a part of the patient as the subject Uses measurable verbs (define, describe, list, walk, demonstrate) Is specific for the patient and the patient’s problem Does not interfere with medical plan of care Is realistic for the patient/patient problem Includes a time frame for patient reevaluation

Role of the LPN/LVN Assessment -Observe/report significant cues Diagnosis -Assist with determination of accurate nursing diagnosis -Gather data Outcomes Identification/Plan -Suggest interventions -Assist with the development of patient-centered goals/outcomes Implementation -Carry out physician/nurse orders -Evaluate effectiveness of nursing activities Evaluation -Assist with reevaluation of the patient after interventions -Suggest alternative interventions when necessary

Nursing Diagnosis and Clinical Pathways A clinical pathway is a multidisciplinary plan that schedules clinical interventions over an anticipated time frame for high risk, high volume, high cost type of cases
