2. Enhancements for PHARMACeHOME 2  There is now an enhancement that allows CPESN pharmacists to record a task each time a CMR is completed in PHARMACeHOME  This will allow the system to keep track of when a CMR was completed and which user recorded the completed CMR task.

3. Enhancements for PHARMACeHOME  Users are still expected to record required documentation: 1.Publish a medication matrix that includes at least two medication lists (one of these lists should be gathered from the patient interview). 2.Record at least one DTP, including the option of entering “none” within the therapeutic considerations section if no DTPs were found 3.Publish a pharmacy care plan/summary 4.**NEW STEP** Send a completed CMR task to yourself 3

4. Enhancements to PHARMACeHOME 4  CMR has been added as a Task type within task pop up window for CPESN pharmacists.

5. Enhancements for PHARMACeHOME 5  Once you have completed all steps of the CMR, send a completed CMR task to yourself, along with any additional information necessary in the Task Instructions
